JOB DESCRIPTION
•Initiate Physical Therapy services upon physician’s referral with applicable diagnosis and orders.
•Interpret available information concerning the individual under care.
•Perform and record initial evaluation and interpret results to determine appropriate care for the patient.
•Develop a plan of care including short and long term goals.
•Implements rehabilitation programs with the use of various therapeutic modalities such as:
1.Superficial Heating Agents like: HOT PACKS, PARAFFIN WAX, INFRARED RADIATION. Deep Heating Agents like: ULTRASOUND.
2.TRANSCUTANEOUS ELECTRICAL NERVE STIMULATOR, NEUROMUSCULAR ELECTRICAL STIMULATOR, TRACTION MACHINE, LOW FREQUENCY CURRENTS.
3.CRYOTHERAPY using ice massage, cold packs and cold baths. HYDROTHERAPY using whirlpool, pool therapy, etc.
•Identify and documents precautions, special problems, contraindications, goals and plans for re-evaluation.
•Re-evaluate, assess patients progress, adjust plan of care when necessary, perform final evaluation and establishes follow-up plan.
•Instruct families in home program prior to patient’s discharge.
•As a member of a medical team, works and assists other health professionals in helping patients with temporary or permanent physical disabilities to achieve the highest functional level and enhance quality of life.
